This CBT Thought Record is a powerful tool designed to help you navigate the complex relationship between your thoughts, feelings, and behaviors. By systematically breaking down a distressing moment, you can shift from feeling “stuck” in an emotion to viewing the situation with clarity and balance.  

Below is a guide on how to complete the Modern CBT Log effectively.


Phase 1: Mindful Observation

The first half of the log is about awareness without judgment.

  • 1. The Situation: Briefly describe the “Who, What, Where, and When.” Stick to the facts as if a video camera were recording the scene.
  • 2. Initial Feelings: This is your moment for Emotional Awareness. Don’t just pick one word; look at the list and see if there are layers. Are you Anxious but also a little Lonely? Be honest about the intensity. Acknowledging that a feeling is a “9/10” isn’t a failure—it’s a brave act of self-recognition.
  • 3. Automatic Thought: Write down the “internal script” running in your head. What is the meanest or scariest thing your brain is telling you right now?

Phase 2: The Investigation

Now, you become a detective looking for “glitches” in your thinking.

  • 4. Identify Cognitive Distortions: Review the 10 common patterns provided (like All-or-Nothing Thinking or Should Statements). Simply naming these patterns can take away their power. When you realize, “Oh, I’m just Fortune Telling again,” the thought starts to feel less like an absolute truth and more like a habit.
  • 5. Alternative Perspective: Based on the facts, what is a more balanced way to see this? If a friend were in your shoes, what would you tell them? This isn’t about “toxic positivity”; it’s about finding a realistic middle ground.

Phase 3: Celebrating the Shift

The final step is the most important for long-term growth.

  • 6. Final Feeling Scale: After reframing, re-rate your intensity. Even a small drop (from an 8 to a 6) is a major victory.
  • The Celebration: If you feel Calm, Empowered, or even just Neutral, take a moment to sit with that feeling. You successfully navigated a difficult internal landscape and came out the other side. Celebrate the fact that you have the tools to change your own emotional state.
